Access to Care Plus Measurable Community Improvement

The Conduent® Healthy Communities Institute platform is a powerful tool for aggregating and analyzing data. With it, Sangamon County in Central Illinois was able to gain deep insights into a growing community health crisis.

Memorial Medical Center (Springfield, IL) recently used the HCI platform and its SocioNeeds Index to get to the heart of issues affecting the local population. Incorporating 150 health and socioeconomic indicators from nearly two dozen sources as well as a countywide survey, HCI was able to focus efforts on one particular zip code.

The Enos Park neighborhood activated their well-organized and active association and, using insights gained through HCI’s analysis, developed an action plan to address specific barriers to care.

In the first year alone, nearly 20% of Enos Park residents received increased access to care, and area hospitals realized a 38% reduction in unnecessary emergency department visits.
